The present work aims at investigating that branch of Telemedicine relying on the Information and Communication Technologies in order to bridge the gap existing among patients, relatives and clinical staff concerning the exchange of information and knowledge among them. More specifically the work focuses on the care of chronic outpatients since because of the distance from specialized centers their clinical state is often overlooked. For this particular kind of pathologies we propose a general approach that provides a highly customizable telemedicine platform supporting physicians, patients, relatives and administrative staff in monitoring physiological parameters for lifelong treatments. Furthermore, the work also investigated distance learning capabilities with the aim of supporting patients in the process of acquiring new information for a better understanding of their condition and an improved management of it.
