Pineal parenchymal tumors (PPT) are a rare and heterogeneous group of intracranial neoplasia. Despite advances in imaging and neurosurgery, data on these tumors remain limited. The aim of this study was to analyze the clinical, radiological, histological and evolutionary characteristics of TPPs. A retrospective bicentric study was carried out over 28 years (1996-2023) in two Tunisian neurosurgical departments, focusing on patients operated on for TPP with histological confirmation. Clinical, radiological, surgical, anatomopathological and evolutionary data were collected and analyzed. The median age was 29 years, with a male predominance. The main symptom was intracranial hypertension. MRI showed homogeneous enhancement for pinealocytomas, and more aggressive aspects for pinealoblastomas. Surgical management of our patients involved treatment of hydrocephalus in ten patients. Biopsy was performed in eight patients. Four pinealocytomas were included, 1 intermediate PPT (grade 3) and 8 pinealoblastomas.
