Matrix metalloproteinases (MMP) collectively known as Matrixins, form a multigene family within metalloproteinases class of endopeptidases. There has been increasing evidence implicating MMPs as key mediators in tissue destruction associated with Periodontitis and Peri-implantitis. Recognition of the need to restore the natural balance between MMPs and their inhibitors lead to the development of a disease management strategy that focused on the modulation of MMPs involved in host response. Therefore recently the development of synthetic inhibitors to block the synthesis of MMPs have been investigated and tested widely. MMPs in gingival crevicular fluid (GCF) or peri-implant sulcular fluid (PISF) are considered to reflect periodontal and per-implant health and disease. Therefore, the aim of present review is to provide comprehensive information on current status of MMPs mediated pathogenesis of periodontal disease, to update various therapeutic methods under investigation for modifying MMP activities as an adjunctive treatment for Periodontitis and peri-implantitis as well as its role to improve current diagnostic procedure.
