Dr. Anne Rose is the Coordinator of the Anticoagulation Stewardship Program at the University of Wisconsin Hospital and Health System (UW Health) located in Madison, Wisconsin. Dr. Rose received her PharmD degree from Butler University. During her post graduate training she completed a PGY-1 pharmacy residency at the Lovell Federal Health Care Center in North Chicago, Illinois. In her current position, Dr. Rose coordinates all anticoagulation related safety, quality, decision support and performance improvement initiatives across UW Health. In addition to coordinating the anticoagulation program, she also practices clinically in the areas of cardiology and cardiothoracic surgery at the University of Wisconsin Hospital and in anticoagulation management at the UW Health Anticoagulation Clinic. In 2011, her Anticoagulation Stewardship Program received the Award for Excellence in Medication-Use Safety from the American Society of Health-System Pharmacists Research and Education Foundation for pharmacist led initiatives in improving patient care.

In the first major section of the book, each chapter focuses on the role of the pharmacist in the management of medication with a specific type of anticoagulants (e.g. warfarin, heparin and target-specific oral agents) in various healthcare settings.  A broader overview of the clinical management of anticoagulation therapy is provided in the second major section, including descriptions of the role of pharmacists in assessing venous thromboembolism risk, ensuring patients receive appropriate prophylactic therapy, and monitoring outcomes.
