This is a selection of papers presented at the meeting of the European Society for Stereotactic and Functional Neurosurgery in 1994 and it gives an update of the state-of-art of treatment of movement disorders, pain and stereotactic techniques. Frameless stereotaxy and the practical usage of a navigator viewing wand system are discussed. A novel approach to the localization of the motor cortex is represented by a description of the usage of 3D MR-imaging. Several papers deal with methods applied for intraoperative monitoring such as microrecording and evoked potentials, and the clinical usefulness of magneto-encephalography is critically evaluated. The revived interest in surgical treatment of movement disorders is represented by a recent study on pallidotomy and there is a review of the experiences with fetal neurotransplantation for Parkinson s disease from one of the leading centers. The long-term results and neurophysiological evaluation of baclofen infusion for spasticity is the topic of two studies. Implantation of chromaffin cells in the spinal canal is a novel approach to the treatment of severe pain which in the future may develop into new modes of pain treatment. All neurosurgeons involved in pain treatment may benefit from the description of CT-guided percutaneous cordotomy and from critical analyses of spinal cord stimulation applied for "low back pain . For the study of mechanisms involved in the spinal cord stimulation effect on peripheral circulation a new experimental, animal model is introduced. The section on pain contains also a survey of the technique, indications and outcome of nucleus caudalis DREZ operations. Any neurosurgeon interested in stereotactic techniques and treatment of movement disorders and pain will find this book useful as it reflects the most recent advances in the field.
