Introduction:  Background. Problems related to total laryngectomy: loss of voice and smell, worsening of respiratory function. Types of voice recovery.- Preparation: Selection of patient to undergo prosthesis implantation (from recruitment to the psychological motivation of the patient). Choice of timing (primary vs secondary). Analysis of the impact factors on positioning, duration and prosthetic function (CH RT pre or post implantation)- Complications assessment related to the choice of timing.- Surgery: Basic surgical steps to ensure optimal functioning of the prosthesis (photos and/or videos). Prosthetic implantation (video and/or images). Detailed analysis of all complications after prosthesis implantation (from normal deterioration of the device, to surgical treatment of complications of tracheoesophageal fistula, closure and resizing of the diameter) and applicable solutions. Outpatient photo and/or video prosthesis replacement.- Rehabilitation: Respiratory, vocal and olfactory rehabilitation. Carmelo Saraniti is currently an Associate Professor of Otolaryngology, Otolaryngology Department, School of Medicine, University of Palermo. His main clinical interest is oncological surgery of the head and neck, especially open and endoscopic laryngeal surgery.

Barbara Verro is currently a resident at the Otolaryngology Department, University of Palermo, Italy. Her primary interest is head and neck cancer.

Simona Fiumara is a speech therapist working on the vocal and pulmonary rehabilitation of laryngectomized patients. She is an expert in all devices for laryngectomy and tracheostomy, and also a technical consultant for a devices company.
