Lesion Sterilization and Tissue Repair (LSTR) represents a novel, minimally invasive approach in pediatric endodontics aimed at managing infected and non-vital primary teeth. Unlike conventional pulpectomy and pulpotomy, which require extensive instrumentation, LSTR relies on chemical disinfection through topical application of Triple Antibiotic Paste (TAP), commonly a mixture of metronidazole, ciprofloxacin, and minocycline. This method effectively eliminates polymicrobial infections within the pulp and periapical tissues, creating a sterile environment that supports natural repair processes and maintains primary teeth until physiologic exfoliation.The dissertation provides a comprehensive review of LSTR's history, indications, advantages, and limitations, along with critical evaluation of experimental studies, clinical trials, and case reports. Evidence demonstrates high clinical and radiographic success rates, with rapid symptom relief, abscess healing, and bone regeneration. Alternative antibiotic combinations, herbal carriers such as aloe vera, and advanced delivery systems have been explored to overcome drawbacks like tooth discoloration and antibiotic resistance.
