Detailed knowledge of the morphology of multirooted teeth and their position in the dental arch is a fundamental prerequisite for a proper understanding of periodontal problems. These problems of multirooted teeth are the result of destructive periodontal disease. They appear as a major challenge to the periodontist because of their unique anatomical characteristics; including the size and shape of the teeth, the root and their alveolar housing, varied nature and pattern of periodontal destruction and their variable responses to treatment. Thus, it creates a situation in which routine periodontal procedures are somehow inadequate and requires special procedures to attain success. For the treatment decisions for the furcation-involved molars, it must be realized that there is no specific scientific evidence that a given treatment modality is superior to the others. However, the key to long term success of treatment of furcation appear to be selection of a well motivated patient with good oral hygiene and a thorough diagnosis leading to careful surgical and restorative management.
